I have two healthy teenage boys - intact. They both have high self-esteem and although it's not a common topic of conversation, they both have been told the story of our decision to not circumcise. God created them perfectly and we didn't want to mess with His perfection; we didn't want them to lose sensitivity; etc. As far as hygiene, during bathtime, I would try to pull their foreskin back to rinse until I read somewhere that like the female genitals - they are "self-cleaning" - so I stopped. My younger son was so happy when I did because it tickled too much! We always have told them they have beautiful bodies and as such, their self-image is intact, as well.
